UNC'S Lineberger Comprehensive Cancer Center joins the Lance Armstrong Foundation's LIVESTRONG Survivorship Center of Excellence Network! Lineberger is the eighth network member institution in the nation and the only one in the Southeast. UNC-LCCC is committed to improving cancer survivorship services for North Carolinians touched by cancer and to accomplish this, we are launching:

OUR MISSION
Our mission is to empower cancer survivors and their families to have
the highest possible quality of life.
You can help us enhance support and educational services for you and your caregivers throughout the cancer journey by taking a few moments to complete the Carolina Well Cancer Survivorship Needs Assessment. Your participation in this survey will help us plan best for the future. All your answers will be strictly confidential. We appreciate your time.
The Carolina Well Program has 3 areas of focus:
1. Establishing a cutting edge survivorship program at UNC-CH,
including both clinical and patient support efforts such as survivorship care
plans to help patients better communicate their needs to their primary care
physicians after treatment, an integrated holistic survivorship care
program, psycho-educational programming, and patient navigation. A website
will be designed to include resources, links, and tools such as decision aids
and tailored feedback for cancer
survivors.
2. Networking with other organizations across the state to help avoid duplication of services, identify current gaps in cancer services, and refer survivors to their own local community resources. There are many great resources in the community. We would like to utilize and improve upon those resources while creating services to fill gaps where support is needed.
3. Partnering with communities across the state to better coordinate services in the communities in which cancer survivors live and work. Current community partners include: Greensboro AHEC Area Health Education Center, Tri-County Community Health Center in Sampson County, and New Hanover Health Network in Wilmington. These community partners will coordinate survivorship programs, offer local educational and training programs for cancer survivors and caregivers including peer counseling, and continuing education for health professionals.
